Proactive Health is looking for dedicated GPs to join our innovative telehealth case conferencing clinic, focusing on chronic disease management for veterans and patients with complex health needs. About the Role
This is not a standard locum position. You will provide clinical leadership within integrated Chronic Care Teams, collaborating with allied health professionals and specialists. The work is structured, purposeful, and genuinely multidisciplinary, prioritizing thorough, evidence-based care.
The role is structured around two dedicated days per week:
- In-Clinic Day: An 8-hour clinic day at our partner practice in Ipswich, focusing on consultations and chronic disease reviews.
- Remote Case-Conferencing Day: An 8-hour telehealth session from your home office, dedicated to structured case conferencing.
There is flexibility in scheduling these two days to accommodate your availability.
Key Responsibilities
As a General Practitioner within our Chronic Care Team model, you will:
- Conduct comprehensive GP consultations for acute, chronic, and complex conditions within a Chronic Care Team framework.
- Deliver preventative health assessments, including cardiovascular risk, metabolic health, mental health screening, and lifestyle risk modification.
- Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
- Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams, integrating various clinical inputs into coordinated care plans.
- Initiate, review, and manage pathology and imaging investigations, provide prescribing and medication management, and make referrals to allied health, specialists, and community services.
- Ensure clinical safety, prioritization, and escalation pathways are clearly identified and actioned.
Requirements
- Medical degree (MBBS or equivalent) and current registration with the Medical Board of Australia (AHPRA).
- Fellowship with RACGP or ACRRM (VR GP), OR eligibility for a 19AB exemption in a DPA location such as Ipswich (international medical graduates are welcome).
- Demonstrated experience in chronic disease management, preventative health, and complex care coordination.
- Proficiency in prescribing, referrals, and investigation management.
- Strong communication and collaboration skills, with the ability to work effectively within multidisciplinary teams.
- For the telehealth component: a suitable home-office environment and a reliable internet connection.
Desirable:
- Experience with telehealth consultations and digital health platforms.
- Experience within team-based or multidisciplinary care models.
- Interest or experience in veteran health, DVA processes, or the Coordinated Veterans’ Care (CVC) Program.
- Familiarity with GP Management Plans (GPMPs), Team Care Arrangements (TCAs), and chronic disease care planning.
