About the Role

VON Canada is seeking a dedicated Case Coordinator to join our Family Supports program. This is a regular, full-time, permanent position offering remote work flexibility. As a Case Coordinator, you will play a vital role in providing comprehensive support to clients, ensuring a high-quality client experience. You will work collaboratively with clients and caregivers to develop personalized care plans, manage care budgets, and navigate essential resources. This role is funded by the Ministry of Children, Community and Social Services and is crucial for delivering successful residential services within our community.

Key Responsibilities

• Conduct comprehensive client assessments using standardized tools (e.g., interRAI CHA, Supports Intensity Scale) and conversational methodologies.
• Develop and maintain therapeutic rapport with clients and caregivers.
• Identify and assess environmental risks to client health and safety.
• Develop and implement individualized care plans in consultation with clients and caregivers, aligning with client goals.
• Facilitate client access to identified services and appropriate community resources.
• Make referrals to community and health resources as needed.
• Collaborate with partners in care to ensure a shared understanding of client needs and appropriate interventions.
• Monitor client progress and adjust care plans as necessary.
• Review client care plan expenditures and support clients in managing their budgets, advocating for additional financial resources when required.
• Collect and report program outcome data to management.
• Adhere to all VON policies, procedures, standards, and privacy regulations.
• Represent VON Canada professionally to clients, external stakeholders, and community agencies.
• Mediate and problem-solve with multiple parties in various contexts.
• Coordinate and provide Family Home support for a caseload in accordance with program guidelines.
• Provide support in emergency and urgent situations.
• Participate in the screening and matching of new Family Home Providers.
• Promote the goals and values of VON and a safe and healthy workplace.

Requirements

• University Degree or College Diploma in a Health or Social Services field.
• Minimum of 3 years of related experience in a home and community care organization or social services program.
• Demonstrated experience in case management and building external and community relationships.
• Proven experience in completing validated common assessment tools and maintaining competency.
• Broad working knowledge of community resources within the defined catchment area.
• Knowledge and skills in comprehensive assessment.
• Strong attention to detail and accuracy.
• Skilled in communication (verbal, written, listening), rapport building, and relationship management.
• Computer proficiency in Windows OS and Microsoft Office programs.
• Ability to use analytical skills and interpret client assessment data.
• Ability to effectively manage multiple tasks, prioritize, meet deadlines, and work well under pressure.
• Strong time management and organizational skills.
• Ability to work independently and as part of a team.
• Ability to accept and facilitate change.
• Ability to maintain confidentiality.
• Must have access to a personal vehicle for business use and possess a valid driver's license and proof of insurance.
• Must be available for weekend assessment coverage and flexible hours, including evenings and weekends, to meet client and family needs.
• Ability to respond to emergency client situations 24/7 (on call).
• A current and original copy of a satisfactory Criminal Records Check is required. A Vulnerable Sector Search and/or a Child Abuse Registry Check may also be required.
