dexter health · Cologne
Job Description
AI Engineer (LLM) - Remote
Company: dexter health Location: Remote Industry: Healthcare Seniority: Mid-level
At dexter health, we are building AI-powered software designed to alleviate the administrative workload in healthcare, enabling caregivers to dedicate more time to patient care. We are seeking a highly motivated AI Engineer to accelerate the development of new AI features and enhance the quality, reliability, and speed of our existing AI workflows.
This is a hands-on engineering role focused on product development, not research or prompt engineering alone. You will be responsible for transforming ambiguous product concepts into functional, production-ready AI features. While the role primarily involves shipping AI product features, it also includes necessary work on AI systems and infrastructure. You will collaborate closely with product and engineering teams, taking significant ownership of your work. We expect you to be comfortable with rapid iteration, making technical decisions in ambiguous situations, and daily utilization of AI development tools such as Claude Code, Codex, Cursor, or Copilot.
Key Responsibilities
- Develop new AI-powered product features from conception through to production.
- Enhance existing AI workflows to improve quality, reliability, latency, and user value.
- Design and implement Large Language Model (LLM)-based workflows, including structured outputs, validation logic, and fallback mechanisms.
- Build evaluation loops, tests, and quality checks for AI-generated outputs.
- Integrate AI capabilities into existing product and backend systems.
- Work with both commercial and open-source LLMs, without being restricted to a single provider.
- Support self-hosted model workflows when beneficial for quality, speed, cost, or control.
- Debug failures in AI features across inputs, outputs, data, backend logic, and user flows.
- Utilize AI development tools as an integral part of your daily workflow.
- Ship features quickly while maintaining high production quality.
Requirements
We value speed, intelligence, ownership, and product impact over formal seniority. We seek individuals who operate with a founder-like urgency, demonstrating the ability to figure things out, make progress under ambiguity, and prioritize outcomes.
Must-Have:
- Strong software engineering skills, particularly in Python or a comparable backend language.
- Proven experience building AI-powered product features or LLM-based workflows.
- Ability to rapidly translate ambiguous product ideas into working software.
- Solid understanding of designing, testing, validating, and improving AI outputs.
- Experience integrating AI workflows into production systems.
- Strong debugging skills across application logic, data, model outputs, and user-facing behavior.
- Experience with evaluation, testing, validation, or quality checks for AI-generated outputs.
- Significant experience using AI development tools (e.g., Claude Code, Codex, Cursor, Copilot) daily.
- High intelligence, rapid learning ability, a bias for action, and an ownership mindset.
- Comfort with ambiguity, making quick decisions, and thriving in a high-trust startup environment.
- Clear written and spoken English proficiency.
Nice-to-Have:
- Experience with frontend or full-stack product development.
- Experience with self-hosted LLMs, model serving, or inference optimization.
- Experience with open-source LLMs and related deployment workflows.
- Experience improving the latency, cost, quality, or reliability of AI systems.
- Experience in healthcare software, healthcare data, or regulated environments.
- Experience with structured data extraction, documentation automation, voice workflows, or workflow automation.
- German language skills.
- Knowledge of nursing homes or elderly care workflows.
This role may not be the best fit if:
- You are primarily seeking an ML research position.
- You are a prompt engineer without strong coding skills.
- You require detailed specifications before starting work.
- You prefer slow-paced, highly structured corporate environments.
- Your primary goal is management rather than hands-on building.
- You intend to use AI tools only occasionally.
- You prioritize perfect processes over fast, reliable execution.
What We Offer
- Remote work flexibility.
- Fair compensation based on skills, experience, and location.
- Ownership of critical backend systems.
- Opportunity to work on software directly impacting real healthcare workflows.
- A modern, AI-native development environment.
- Significant opportunities for professional growth within the company.
